Condition for signing the health care contract is the delivery of confirmation of the patient´s removal from the register of your previous dentist or an affidavit that the patient is not registered at another doctor.
If patient is insured in the Union or Všeobecná zdravotná poisťovňa health insurance company, patient has a signed contract for health care with our clinic thus meeting the statutory terms and condition so patient will be charged the fees that are in the column "WITH A CONTRACT" of the actual price list.
Discounted rates are distinguished by colour in the price list.
Our patients do not have to be insured in any health insurance company and they do not have to sign the contract on health care with our clinic if they are insured.
In this case, we do not require medical documentation from a previous dentist or a certificate of the patient´s removal from the register.
Non-contractual patients are charged the fees that are in the column "WITHOUT A CONTRACT" of the actual price list.
Patient is always informed about the diagnosis, treatment options with alternatives, treatment procedure and treatment costs and duration, before the treatment.
Patient may choose an alternative that suits him/her best. The cost of treatment is determined based on the actual price list.

In order to provide services with maximum quality we use a reservation system for a specific date and time. This way of working relieves the patient from unnecessary waiting in the waiting room and provides the doctor with a time convenient for the treatment.
The best way to arrange the visit's term is by phone, during treatment hours or by leaving a message on the answering machine. Failing to appear or cancelling the visit within less than 24 hours before the booked date is charged.

The patient may request a copy of the medical documentation provided in the form of a copy certified by the doctor.
A copy of the medical documentation can be picked up by the patient, his/her legal representative, or authorized person with the authorization to perform the collection of the medical records with the signature of the patient certified by a notary.
Releasing a copy of the medical documentation is charged under the actual price list.
